Chapter 9 : Appendices for Self-Study Report

The appendices should contain all the supporting data and information cited in the Self-Study, including:

  • Curriculum design and delivery review related appendices listed in the curriculum design and delivery review guide (guide #2)
  • Calendar description of program
  • Course descriptions (not outlines)
  • Program admissions requirements/policies
  • Faculty Bios (not CVs)
  • Internal data (enrolment, retention, graduation, grade distribution, etc.)
  • Student survey results
  • Faculty survey results
  • Alumni survey results
  • Industry/discipline survey results
  • Other surveys, data or information cited in the report

Appendices must be submitted as a separate document. Each appendix must be labeled with a letter (A, B, C, etc.) followed by a descriptive title and arranged sequentially by the order they were first referenced in the report.
