Chapter 9 : Appendices for Self-Study Report
The appendices should contain all the supporting data and information cited in the Self-Study, including:
- Curriculum design and delivery review related appendices listed in the curriculum design and delivery review guide (guide #2)
- Calendar description of program
- Course descriptions (not outlines)
- Program admissions requirements/policies
- Faculty Bios (not CVs)
- Internal data (enrolment, retention, graduation, grade distribution, etc.)
- Student survey results
- Faculty survey results
- Alumni survey results
- Industry/discipline survey results
- Other surveys, data or information cited in the report
Appendices must be submitted as a separate document. Each appendix must be labeled with a letter (A, B, C, etc.) followed by a descriptive title and arranged sequentially by the order they were first referenced in the report.
